Intake, consent, authorisation and administrative forms — built in the app, filled in by the patient, and filed against their record without anybody re-typing anything.
Intake forms, consent forms, authorisation forms and the administrative paperwork a front desk hands over. The builder takes fields, headings, checkboxes and signatures, and shows the finished form beside you as you work.
Specialty starting points come with the clinic type — a behavioral-health intake reads differently from a dental one, so they are not the same form with the title changed.
Text a patient a link and they fill it in on their phone. Their answers reach us on the way to you, are attached to their record, and are then erased — as soon as your practice has them, and within thirty days regardless.
This is the one thing in LogBlues that travels through our servers at all, it is off until you switch it on, and it is the only exception to everything on the privacy page.
Every form prints with the clinic name, address and logo if you want them, or plain if you do not. The footer and the optional notice are yours to set.
