A return visit may begin with a new symptom, a hospital discharge note and a medication list that no longer agrees with the chart. LogBlues keeps the reconciliation, diagnostic question, outside records and follow-up actions connected to that encounter.

When results arrive, they return to the same patient timeline and the responsible clinician receives visible work. The next visit begins with what changed, not another reconstruction of the story.

The work

What happens in LogBlues.

The active problem list stays readable, so the clinician can separate what is stable, what changed and what needs a decision today.
Medication reconciliation is visit work, with additions, discontinuations and safety questions documented beside the reason for each change.
Labs and diagnostics form trends, instead of returning as unrelated files that have to be remembered one at a time.
Specialist referrals remain tied to the clinical question, so the consultation, report and resulting plan stay in one case.
Care plans carry ownership and dates, for monitoring, repeat testing and the next conversation with the patient.
Services become visit charges, so a complex office visit and each performed test reach checkout and Billing together.
