Adult medicine becomes difficult where diagnoses, medicines, specialists and results overlap. The record has to show those relationships without flattening a complex patient into a problem list.
Setup
The Internal medicine profile brings chronic conditions, medication reconciliation, diagnostic workups, specialist coordination and longitudinal care plans forward from the start.
From start to finish
A return visit may begin with a new symptom, a hospital discharge note and a medication list that no longer agrees with the chart. LogBlues keeps the reconciliation, diagnostic question, outside records and follow-up actions connected to that encounter.
When results arrive, they return to the same patient timeline and the responsible clinician receives visible work. The next visit begins with what changed, not another reconstruction of the story.

The design point
Internal medicine depends on seeing interactions: one medicine changes a lab, one specialist recommendation changes another plan, and one missed follow-up leaves several conditions exposed.
A connected timeline, cases and assigned work make those dependencies visible while preserving the distinct documentation for each encounter.
The result is not a shorter chart. It is a chart that can be read in the order care actually happened.
