The work

- Each resident has one continuing record, rather than a new story for every staff rotation.
- Medication administration is documented in sequence, so the next shift sees what was actually recorded.
- Shift notes preserve the handoff, with tasks carrying unfinished work to a named person.
- Incidents open a visible thread, for documentation, files and the follow-up they require.
- Family communication stays on the resident timeline, instead of disappearing into a personal phone.
